Personal Data Agreement

Consent according to the EU Data Protection Regulation (DSGVO) and the Federal Data Protection Act (BDSG) Data Protection Germany from May, 2018.

Personal data agreement with the International Academy of Veterinary Chiropractic (IAVC),

in particular name, address, telephone number and e-mail address, which are necessary for the sole purpose of carrying out the contractual relationship, shall be based on statutory rights.

Any further use of personal data and the collection of additional information requires the consent of the person concerned.

I agree that the International Academy of Veterinary Chiropractic saves my personal data for the purpose of contacting me concerning all matters related to attending their courses.

I agree that the International Academy of Veterinary Chiropractic saves my personal data for the purpose of contacting me concerning FUTURE courses, to inform me by e-mail, telephone, fax or post, as well as be named as an IAVC graduate, with a class photo on the IAVC website. This may include other photos taken for promotional or teaching purposes.

I have read the following, and understand as it pertains to my rights.

In accordance with Article 15 of the GDPR, you are entitled at any time to request information from the International Academy of Veterinary Chiropractic (IAVC) about your saved personal data. Pursuant to Article 17 of the GDPR, you are eligible at any time to request that the International Academy of Veterinary Chiropractic correct, erase and block individual personal data. In addition, you can make use of your right of objection at any time without stating reasons and amend or revoke the given declaration of consent with effect for the future. You can submit the cancellation either by post, by e-mail or by fax to the contracting party (IAVC). You will incur no other costs than the postage costs or the transmission costs according to the existing base rates.
